Check the official Phuket office channel
Phuket Provincial Land Transport Office is Smart Queue site 84. On 20 August 2026 it was selectable and exposed ‘Foreigner: NEW THAI DRIVING LICENCE’. That is stronger than office visibility alone, but it does not confirm that a foreign-licence conversion or a date is available.
Confirm conversion, not a first-time test slot
Describe the appointment as a first Thai licence based on a valid foreign licence for the same vehicle type. Ask whether car and motorcycle applications can be handled together and which document set each requires. Do not assume a generic 'new licence' label explains the shorter route.
Plan language and transport without guarantees
English support may be available, but it is not a published entitlement. If you cannot confirm details in Thai, ask a Thai speaker to help. Leave a realistic travel buffer, but ignore private claims that every Phuket conversion is a walk-in or always finishes the same morning.

The published operational checklist asks for at least 15 days remaining, and the foreign licence should remain valid for the full authorised Thai stay. Confirm near-expiry cases before crossing the island.
Certificate of Residence or office-confirmed alternative
Phuket Immigration publishes a residence-certificate form with Driving License as a stated purpose.
Its main office is at 482 Phuket Road, Talat Yai, Mueang Phuket 83000, phone 076 221 905; published hours are Monday–Friday, 08:30–12:00 and 13:00–16:30. The official pages do not confirm that Patong or Blue Tree satellites issue this certificate, so use the main-office contact before attending.
Medical certificate
The official checklist gives the medical certificate a 30-day validity window from issue.
Clinic charges are separate. Obtain it close to the confirmed appointment, not months in advance, and ask whether separate originals are needed for car and motorcycle applications.
Valid foreign licence, translation if needed, and training result
The original licence must cover the requested vehicle type.
If it has no English, bring the Thai or English certification required by the regulation. Complete the one-hour foreigner e-learning and keep both digital and paper evidence in case the office uses a paper workflow.

A matching category is especially important for car versus scooter applications
The same national testing table applies in Phuket: accepted same-type foreign evidence generally waives the written and driving tests, while physical screening remains. The office must be able to verify the category first.

Submit originals and copies
Staff check passport, residence, medical, training, licence validity, language, and category.
Present the car and motorcycle entitlements separately. A rental shop's willingness to rent a scooter does not prove that a car-only foreign licence supports a motorcycle conversion.

Complete reaction and vision screening
Expect the standard physical-fitness checks, commonly including reaction and several vision functions.
These checks are not the computer theory test and not the practical driving course. Follow the station instructions and ask for clarification if the language is unclear.

Confirm why any full test is requested
If the office assigns theory or practical tests, ask whether the foreign category, validity, translation, or another document failed the conversion check. A standard new motorcycle licence does require its own test route; a car licence cannot remove that requirement for a scooter.

Pay and receive the approved Thai category
After approval, screening, fee payment, and the office photo, DLT normally issues a 2-year temporary Thai licence for the accepted category.
A car and motorcycle remain separate licences. The registrar can still request more evidence, medical confirmation, or a driving retest after reviewing the individual case.

Separate the official licence fee from everything needed to reach the counter with an accepted file. That keeps private service prices and government charges from being confused.
Official DLT counter charge
The official checklist lists 205 baht for a private car licence and 105 baht for a motorcycle licence, including the request fee. Two categories mean separate licence charges and may mean separate supporting originals.
Variable Phuket expenses
Budget separately for the medical certificate, residence evidence, translation or certification, photocopies, photos if requested, and transport to the provincial office. Those costs depend on provider and timing; the DLT does not publish one Phuket all-in total.
Keep optional document help separate
Translation, certification, and practical document help are not part of the DLT tariff and cannot create a missing vehicle entitlement. Confirm the official process first and keep those optional costs separate from government charges.
